The engineer involved is treated with insulin for type 1 diabetes, and hypoglycemia &#8211; with symptoms including drowsiness, difficulty concentrating, and confusion &#8211; is common for those so treated. No medical assessment was conducted following the collision, so the TSB investigation could not definitively determine the cause of the cognitive decline.<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">According to <a data-ylk=\"slk:the investigation repor;elm:context_link;itc:0;sec:content-canvas;\" data-yga=\"{&quot;yLinkElement&quot;:&quot;link&quot;,&quot;yLinkElementType&quot;:&quot;article_link&quot;}\" href=\"https:\/\/www.tsb.gc.ca\/eng\/rapports-reports\/rail\/2023\/r23e0079\/r23e0079.html\" rel=\"noopener nofollow\" target=\"_blank\">the investigation repor<\/a>t, the TSB found that there had been no diabetes assessment for the engineer, as required by the Canadian Railway Medical Rules Handbook, for the years 2011, 2012, 2013, 2017, 2020, 2022, and 2023. Assessments for other years were incomplete but had been accepted by CN. The TSB also determined that Transport Canada had not conducted audits to verify CN compliance with medical rules in the 12 years prior to the incident.<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">As a result, the TSB is recommending that Transport Canada establish regular oversight activities to assure compliance with railway medical rules. TSB<\/p>\n<p><\/span><\/div>\n<\/figcaption><\/figure>\n<hr class=\"my-4\"\/>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">The collision occurred about 10:27 a.m. at Mile 141.9 on CN&#8217;s Wainwright Subdivision, just west of Wainwright on CN&#8217;s main line between Saskatoon, Saskatchewan, and Edmonton.<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">As westbound CN train 115 approached the site of a planned crew change at Wainwright West on the north main track, the engineer trainee was operating the train when the conductor called signals at miles 136.9 and 139.4; the other crew members did not respond. Train 115 passed a Clear to Stop indication at mile 139.4 at 42.2 mph. The trainee asked shortly thereafter if the train was approaching its crew change point too fast; he subsequently asked the engineer for direction on five other occasions before making an emergency brake application just 400 feet before passing a stop signal, beyond which another train was crossing onto the north main track. Train 115 was going 20.4 mph when it collided with the side of the other train, 771, which was moving in the same direction at 10.7 mph.<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">Both locomotives and two double-stack cars derailed on train 115; six loaded sand cars derailed on train 771. There was a fuel spill from the lead locomotive on train 115, but no fire. There were no injuries and no other hazardous materials involved.<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">CN, as a result of the accident, added a special instruction to its rule regarding Clear to Stop signals requiring trains to reduce speed to 10 mph below permissible track speed before passing a Clear to Stop indication. It also issued a bulletin regarding crew actions in Critical Focus Zones, which cover areas 3 miles from a Stop signal indication, or from the moment such a signal is observed, if it is within 3 miles.<\/p>\n<p class=\"mb-4 text-lg md:leading-8 break-words\">The TSB report notes that it has previously called for implementation of enhanced train control, which would address crew failures to respond to signals. It has also asked that Transport Canada adopt interim measures to mitigate the risk of crews not complying with signals.
